The tax form you fill out is based on the event(s) held by your organization. The home page of the site has links to four separate Tax Forms:
- Weekly Beano Tax Report - Please complete this form each week if your organization is an active Beano licensee, even if an event was not held for the week in question.
- Annual Beano Tax Report - Please complete this form at the beginning of the calendar year to account for all revenue and expenses throughout the previous calendar year.
- Raffle Tax Report - This form should be completed if your organization recently operated a Raffle in the commonwealth of Massachusetts.
- Casino/Monte Carlo Night Report - This form should be completed if your organization recently operated a Casino Night in the commonwealth of Massachusetts.
